Sliding Fee Program

The Sliding Fee Program is a federal program that reduces fees charged to patients based on their income and family size. The purpose of this program is to provide patients with the opportunity to access primary health care. The eligibility requirements are based on the federal poverty guidelines published annually by the U.S. Department of Health and Human Services.

HOW TO APPLY

Patients must complete and submit the sliding fee application, which can be obtained at the front desk at any of our three locations. The application can also be found here.

DO YOU QUALIFY?

For families/households with more than 8 persons,

add $4,720 for each additional person.

 

TO MAINTAIN SLIDING FEE STATUS:

  • Payment is expected in full when services are rendered.

  • The sliding fee DOES NOT allow balances.

  • You must update all paperwork yearly.

Stedman-Wade Health Services Inc. is a FTCA Deemed Facility. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
