Wade Family Medical Center provides integrated behavioral, substance abuse and medical healthcare featuring treatment of opioid use disorder and other addictions.
We have integrated behavioral health services in our primary care offices to further meet the health needs of the patients we serve.
The goal of behavioral health integration is to provide access to physical and behavioral health concerns and conditions. The Licensed Clinical Social Worker and Primary Care Providers work together to address behavioral health needs.
The members of the team support patients, families and caregivers with self-management, self-efficacy and behavior change.
